Rent a BD/Carefusion/Pulmonetic LTV 1200 Ventilator

This is a rental unit and not for the purchase of an actual unit. You will receive the product for the amount of time you paid for. In order to rent from us, you will have to fill out a form that will be emailed to you when you purchase the rental.

RR Rental Requirement 

1. -Copy of a valid US ID.
2. -Copy of an official mail showing you name and address.
3. -ACH banking Info for the name on the rental order to bill monthly rental fee.
4. -Copy of your Credit Card as backup to bill the monthly rental fee in the case of Failed ACH Payment. 

A deposit may be required upon rental and will be refunded upon the return of the unit.
